
College Scholarship Instructions and Application
The scholarship application will be open from December 9, 2024 through April 4th, 2025.
The Slocum Research & Education Foundation’s mission is to improve musculoskeletal care across the lifespan through community-based research, education and collaboration. Supporting the development of future healthcare professionals through our annual college scholarship program is one way we work to achieve this mission. The Scholarship Program targets well-rounded high school seniors who plan to pursue a career in healthcare, health-related research, public or community health. The purpose of our Scholarship Program is to enable qualified students to pursue their educational goals. A one-time, minimum of $1,500 scholarship will be awarded to the most competitive applicants as evaluated by the criteria below.
Selection Criteria
The selection criteria include: Academic achievement, involvement in extracurricular activities, leadership activities, dedication to community service, outstanding letters of recommendation, and the need for financial support to pursue educational goals. Applicants with a grade point average under 3.0 or those who do not plan to pursue a health-related career are unlikely to receive support. Students must reside in Lane County and be accepted to an accredited college or technical school program in the United States.
Required Application Materials
Most recent complete high school transcript.
One professional or academic resume demonstrating paid and volunteer work experience, participation in extracurricular activities, academic honors and achievements, and community service/volunteerism. Please note, you must include the duration and frequency of participation in extracurricular and volunteer activities. Here are acceptable formats to include duration and frequency of activities for your scholarship application:
List as part of your resume (upload one document)
Include a list used for a prior scholarship or college admission application (upload two documents = resume and existing list)
Create a grid (link to Oregon Goes to College fillable gird) and populate all volunteer and extracurricular activities (upload two documents = resume and new grid)
One 750 word essay (please refer to guidance below)
Two letters of recommendation, one of which must be from a teacher or academic advisor/counselor. Letters of recommendation will also be submitted electronically through this website. Please direct your teacher and/or academic advisor to click here to submit. You may also use the navigation button below to access the letter upload page. Please note all letters must be dated and signed before submission. Letters without a signature will not be accepted.
If you submitted a FAFSA application please submit your expected family contribution (EFC) as generated on your FAFSA. Regardless of FAFSA status, please submit your estimated household income.
Please note, you must organize your material in advance of completing the submission process below. You cannot start, then save your submission and return to the application at a later date. All form fields must be completed and materials uploaded at the same time.
Essay Guidance
The Slocum Foundation’s vision is for healthcare providers to base clinical decision making on the best available evidence, tailor prevention and treatment plans to meet the needs and goals of individuals, and invest in solutions for improving musculoskeletal outcomes across care settings and in our communities. In no more than 750 words, describe your vision for a health-related career. Tell us how your life experiences have influenced your educational path, and detail your educational plans after high school.
Award Notifications and Reception
Applications must be received by Friday, April 4th, 2025. Award recipients will be notified by May 2nd, 2025. Award recipients will be acknowledged at the Slocum Foundation’s annual Scholarship Award reception held on May 13th, 2025 at 5:30pm. Award reception at the Slocum Center - 2nd Floor Atrium.
