Slocum Orthopedics Emerging Health Professionals of Lane County Scholarship Program

The Slocum Orthopedics Emerging Health Professionals of Lane County Scholarship Program aims to support future healthcare professionals by awarding a limited number of one-time, $1,500 scholarships to graduating Lane County high school seniors who have been accepted into an accredited college or technical program. The Scholarship Program is focused on supporting well-rounded high school seniors who plan to pursue a career in healthcare, health-related research, public or community health.

The scholarship application will be open from December 15, 2025 through March 29th, 2026. Please review the selection criteria and required application materials below before beginning your application.

Selection Criteria

Applications will be reviewed using the following criteria:

  • Clear goals for a health related career path are stated.

  • Evidence of academic achievement, involvement in extracurricular activities, leadership activities, and dedication to community service is demonstrated.

  • Outstanding letters of recommendation are provided.

  • Applicants with a grade point average under 3.0 are unlikely to be selected.

  • Those who do not plan to pursue a health related career will not be selected.

  • Applicants must reside in Lane County.

  • Applicants must be accepted to an accredited college or technical school program in the United States. (acceptance status will be verified prior to award distribution)

Required Application Materials

Please complete all of the required application materials prior to beginning the submission process. The application form cannot be saved and completed at a later time.

  1. Most recent complete high school transcript

  2. Resume demonstrating paid and volunteer work experience, participation in extracurricular activities, academic honors and achievements, and community service/volunteerism.

  3. Completed Oregon Goes to College Activities Chart linked here. Please complete this grid digitally and resize columns as needed so that all information is visible to reviewers. Download the completed chart as an excel spreadsheet or PDF for submission.

  4. One 750 word essay. In no more that 750 words, describe your vision for a health-related career, how your life experiences have influenced your educational path, and how your educational plans after high school will help you to pursue this career path.

  5. Two letters of recommendation. At least one of the letters must be from a teacher or academic advisor/counselor. Letters will be submitted electronically using the form at the bottom of this page. Please direct your recommenders to the page to submit. Please note all letters must be dated and signed before submission. Letters without a signature will not be accepted.

  6. If you submitted a FAFSA application please submit your Student Aid Index (SAI) as generated on your FAFSA. Regardless of FAFSA status, submit your estimated household income.

Award Notifications and Reception

Applications and letters of recommendation must be received by Sunday, March 29th, 2026. We will reach out with the results of your application in late April or early May.

Award recipients will be acknowledged at the Slocum Foundation’s Annual Scholarship Award reception held on May 12th, 2026 at 5:30pm. Award reception at the Slocum Center - 2nd Floor Atrium.
